Welcome to the 5th Avenue Project

The 5th Avenue Project offers holistic and responsive support for neurodivergent individuals and their families. Our aim is to remove barriers, avoid long waits and provide a single place where families feel understood, supported and empowered.

Referral Pathway

How our referral pathway works

Initial contact
Referrals usually come through the Great Minds Together website or by contacting our admin team by phone or email. From the first contact we focus on listening and understanding what you need.


Initial triage
Our admin team gathers key information about the individual, any urgent needs and the main areas where support is required. This helps us understand priorities and the scope of support.


Capacity and skills matching
We then look at the capacity and skills within our Intensive Support Practitioners and managers, and match the referral with the person who is best placed to help. This may be education focused, practical, therapeutic or advocacy focused.


Allocation
Once the right practitioner or manager is identified, the case is allocated straight away. Our commitment is to offer prompt contact and engagement, in line with our promise of no waiting lists.

Our Support in Practice

Once a case is allocated, the named practitioner becomes the main contact for the individual and family. They coordinate our wraparound offer and bring together the different elements of the 5th Avenue Project.


Learning and development

  • Learning hub
    Access to online resources, workshops and materials that build understanding, confidence and strategies for everyday life.
  • Growth Mindset programme for young people
    Activities and approaches that encourage resilience, perseverance and a positive approach to challenges and change.
  • OPTIONS programme
    Support for decision making, future planning and exploring possibilities that are meaningful for the individual.
  • Life skills curriculum
    Practical sessions that focus on real world skills and increased independence, tailored to each person and family.

Practical and essential support

  • Food bank and uniform bank
    Discreet and timely access to essential items for families who need this support.
  • Benefit forms
    Guidance through benefit applications, including understanding eligibility and support with paperwork.
  • Housing support
    Advice, advocacy and practical help when families are facing housing challenges.

Advocacy and coordinated support

  • Education, Health and Care Plans
    Support across the full EHCP journey, from first application through to reviews and appeals, including attending meetings and advocating for the child or young person.
  • Coordination of services
    Acting as a central point of contact, working alongside schools, health teams, social care, therapists and community groups to create an integrated support plan.
  • Signposting
    Linking families with local and national specialist services, charities and networks that can add to their support.
