
The 5th Avenue Project offers holistic and responsive support for neurodivergent individuals and their families. Our aim is to remove barriers, avoid long waits and provide a single place where families feel understood, supported and empowered.
Initial contact
Referrals usually come through the Great Minds Together website or by contacting our admin team by phone or email. From the first contact we focus on listening and understanding what you need.
Initial triage
Our admin team gathers key information about the individual, any urgent needs and the main areas where support is required. This helps us understand priorities and the scope of support.
Capacity and skills matching
We then look at the capacity and skills within our Intensive Support Practitioners and managers, and match the referral with the person who is best placed to help. This may be education focused, practical, therapeutic or advocacy focused.
Allocation
Once the right practitioner or manager is identified, the case is allocated straight away. Our commitment is to offer prompt contact and engagement, in line with our promise of no waiting lists.
Your information helps us understand what you need and match you with the right member of our team.

Once a case is allocated, the named practitioner becomes the main contact for the individual and family. They coordinate our wraparound offer and bring together the different elements of the 5th Avenue Project.
