Referral Form

Great Minds Together supports children and young people with special educational needs and disabilities and/or social, emotional, and mental health needs. We provide a free wraparound support service to children and young people under the age of 18 (up to 25 with an EHCP) and this support also extends to their families.  Support includes but is not limited to: mentors for both adults or children/ young people and Wraparound Coordinators to help manage and coordinate contact with services. 

We offer a holistic service with a multidisciplinary team so are able to provide both virtual as well as practical support where appropriate. Great Minds Together supports families with all aspects of their education including applications for EHCPs as well as help to access housing, benefit advice, parenting advice, signposting and mediation services, etc.   

Due to the need for our Outreach services continuing to grow, we will now review on a monthly basis. The next review will be on the 30th December 2024 for West Yorkshire, if necessary we will resume the pause.

UPDATE

Referrals in West Yorkshire local Authorities are open as usual for Outreach support.

Referrals in Greater Manchester remain on pause, this is due to a high volume of referrals received throughout October. However, we have now implemented a waiting list, so you are welcome to submit your referral for it to be added to the waiting list and this will be reviewed in the new year.
